Healthcare AI spent the last two weeks looking less like an experiment and more like plumbing, even as the people building the frontier models argued about who should govern them. The loudest signal came from outside medicine. In the span of about five weeks, the CEOs of Anthropic, OpenAI, and Google DeepMind each published frameworks that converge on the same prescription: independent third-party testing before a model reaches the public, and a single U.S. body to certify it rather than a patchwork of state laws. Demis Hassabis' version drew praise from Sam Altman and Satya Nadella, and Anthropic's Jack Clark called it "excellent." The backdrop is that Washington improvised restrictions on frontier models twice this summer, and critics warn the emerging rules could entrench the biggest labs. For clinical AI, the logic underneath is familiar: test it hard before it touches a patient, then certify what works.

Hospitals are already living that logic. Mayo Clinic now runs about 150 AI models across its system, including a tool called Record Time that trims 5 to 30 minutes off a physician's chart review before each visit. Mayo vets each tool the way it runs a clinical trial, starting small under supervision and treating the adoption rate as the real measure of success. The trust question has not gone away, though. Mayo's former research operations director is suing the system over privacy and oversight concerns tied to some of its AI.

The rest of the sector has moved past pilots too. CIOs at large systems now describe AI as infrastructure rather than a pilot program, with enterprise deals spanning documentation and triage. The clinic-facing payoff still lags wherever reimbursement doesn't follow. A new AMA and Medscape survey found 86% of U.S. physicians review patient wearable data while only 6% have it wired into their workflow, because no dedicated Medicare pathway pays for reviewing consumer-device data. AMA CEO John Whyte put it plainly: the data is available but not actionable. By comparison, 35% of German physicians use DiGA, that country's reimbursement route for approved health apps.

Diabetes startup UpDoc revealed the first clearance for a device built around a patient-facing large language model, an insulin-management tool that took the low-friction drug-dose-calculator route rather than the novel-device pathway, a choice other developers are likely to copy. Payment Fights on Every Front

Almost every fight in Washington this fortnight came down to who pays and how much. Start with physicians. Days after CMS previewed another round of 2027 reimbursement cuts, the leaders of the GOP and Democratic Doctors Caucuses introduced the Patients First Act, which would tie Medicare physician pay to inflation by setting annual updates at the Medicare Economic Index minus one point and capping year-to-year swings at 2.5%. It would replace the MACRA-era formula the AMA blames for a 33% inflation-adjusted pay drop since 2001, and it frames the fight as an anti-consolidation measure, since squeezed independent practices tend to get bought by hospitals. Provider groups lined up behind it fast.

Hospitals, by contrast, got hit. In its proposed 2027 outpatient rule, CMS moved to slash 340B drug reimbursement from average sales price plus 6% to average sales price minus 33.4%, roughly a 37% cut, while expanding site-neutral payment to imaging done in off-campus hospital departments. The agency also proposed accelerating its clawback of a prior $7.8 billion 340B remedy by raising the annual conversion-factor offset from 0.5% to 3%. The AHA called the package destructive and warned it lands as hospitals absorb more uninsured patients.

Patients are feeling it too, and it has become a live political problem. House centrists led by New York's Mike Lawler kept pressing leadership to address expiring ACA subsidies, an affordability fight both parties are now racing to own ahead of the midterms. The numbers explain the urgency. KFF's read of preliminary filings shows ACA insurers seeking a median 14% premium increase for 2027, a second straight double-digit year that would push typical premiums up more than a third across two years. Enrollees above 400% of the poverty line, who lost subsidies entirely, absorb the full hit.

Oversight had its moment as well. Erica Schwartz, President Trump's third CDC nominee in under two years, told the Senate HELP Committee she would "never betray the science" and pledged radical transparency to rebuild trust in the agency. Senators from both parties pressed her on whether she would resist pressure from Secretary Kennedy, and she declined to commit to removing a CDC webpage that entertains a debunked vaccine-autism link. Schwartz, currently a UnitedHealth executive earning about $850,000, is still expected to clear.

And the medtech world got its next regulatory roadmap: FDA and industry released a draft MDUFA VI commitment letter setting device-review goals for 2028 through 2032, adding a faster "Focused Follow-Up" pre-submission track and trimming the 510(k) decision goal to 112 days by FY2032, with a public meeting August 5 and a final letter due to Congress by January 15.

The Checkbooks Come Out

One theme ran through the past two weeks: capital moving, and moving fast. Venture money is flooding back into health tech, with digital health startups raising $7.4 billion in the first half of 2026, a billion more than the same stretch last year on roughly the same number of deals. Rock Health credits an AI-powered rebound, though the concentration is stark, with 20 megadeals of $100 million or more absorbing 45% of all the capital. Telling detail: the firm has stopped tracking "AI-enabled" as its own category, because it now describes almost everything.

The marquee round went to preventive care. Neko Health, the full-body-scan startup co-founded by Spotify's Daniel Ek, raised $700 million in a Lightspeed-led Series C that values it near $7 billion, roughly four times its valuation a year ago. The company sells a radiation-free scan that captures millions of data points, and it plans to open its first U.S. clinic in New York later this year with a waitlist already past 350,000.

Pharma kept its checkbook out. Eli Lilly agreed to buy AtaiBeckley for about $2.8 billion upfront, plus up to $1 billion tied to milestones, its first move into psychedelics. The prize is BPL-003, a DMT-based drug in Phase 3 for treatment-resistant depression that could be dosed as little as twice a year. It is Lilly's eighth acquisition of 2026, a year in which it has already pledged north of $10 billion upfront, and it lands just weeks after AbbVie entered the same space.

Devicemakers were busy too. A PwC tally counted $36.5 billion in medtech deals in the first half, led by Boston Scientific's $14.5 billion purchase of Penumbra and Danaher's $9.9 billion buy of Masimo, with Medtronic alone appearing in three of the ten largest as it went "back on offense." The money is clustering in faster-growing categories like structural heart and electrophysiology.

Then there was the payer that reminded everyone why margins matter. UnitedHealth posted $5.48 billion in second-quarter profit and raised its full-year outlook, as its medical cost ratio fell to 86.7% from 89.4% a year earlier. The rebound came with a shrinking footprint. UnitedHealthcare covered 48.5 million people, down 525,000 in a quarter, as it exits unprofitable Medicare Advantage and ACA markets and pours $1.5 billion into AI. The CFO cautioned that costs remain elevated.
